Looking for best PDOC in San Francisco

hi,
i am exploring changing my pdoc as it has been a few years and have not gone into remission and while he seems to be well regarded he is also extra-conservative and there have been some inconsistencies with what he has been suggesting vs. things i've seen from my own research.

currently on nardil and thus looking for a top-flight pdoc or psychopharmacologist in san francisco (who obviously has experience with MAOIs since that is where i am at this point).

if any one has any guidance or suggestions or names please babblemail me. would rather go to someone in the community instead of ringing up UCSF and becoming a clinic patient.

desperate here. been suicidal off and on for months and unemployed and i just dont know how much longer i can take this. have had repsonses to SNRIs but they were transietn and hypomanical, so i am still putting my faith in meds instead of ECT, but like i said i am at my last rope.

You're not going to become a clinic patient, if you mean a guinea pig -- if you're in a unique situation in a metropolitan area that has three UC institutions and I forget the number of Cal State, academia is where you are going to find the best psychiatrists because they have been in ER rotations where they have seen many cases before being in clinical practice.

I would suggest doing exactly what you seem to not want to do:

That's where you find people who work in academic circles and have many colleagues and you will find the best psychopharmacologists that way. Its how I have been treated for years.

I'm going to echo yxibow, albeit somewhat less enthusiastically. UCSF is the very first place I'd call, if I were still in the City and didn't have a good referral from a trusted source. Even if they were not the right solution for me, they'd be a good starting place -- and they may be able to offer some referrals. My lack of enthusiasm is based on having received ALL my health care through UCSF until I was in my 20s -- my mother believed very strongly that the best care available was through a teaching hospital. Private doctors were a revelation to me when I first saw one -- "wow! I didn't have to wait an hour past my appointment time!" Still, UCSF is the place to go if you're having trouble finding what you need.

Otherwise, NAMI is a good choice. If you have a good GP, that's a great place to get a referral, too. My first choice is to ask a psychotherapist -- if you don't have a therapist yourself, call around and ask anyway. Find a friend in therapy with someone he/she likes, and ask that therapist. The benefit of asking a therapist is that they'll have joint clients, and will have a good idea of how well patients respond to the various candidates.

I'm not a fan of Kaiser, and you'd have to be a member anyway, but that's also my own bias. If you have transportation and don't mind the hike, Palo Alto Medical Foundation will likely have someone good, or -- and this would be my other choice, before PAMF, but again that's just my bias -- Camino Medical Group. Both CMG and PAMF are contracted with a number of insurance plans. If you're uninsured, another option is to call the Haight Ashbury Free Clinic and ask if there's anyone they recommend. (They're good people there, and you might also give them a try, if there are economic issues at play.)
