Need help want depression and impulses in control

Hi, I have a diagnosis of bipolar (with the possible differential of schizoaffective). For years I suffered from severe depression with extremely violent rage attacks. I'm right now taking Trifluoperazine 20mg per day divided with gabapentin 800mg (which doctor raised yesterday). I cannot tolerate most atypical antipsychotics or most conventional mood stabilizers like lithium, valproate, carbamazepine. I can seem to tolerate standard APs like haloperidol and trifluoperazine so I'm looking for standard APs. Actually, doctor won't give haloperidol long term and trifluoperazine imperfectly controls rage impulses and attacks though it clamps it down a little bit. The problem also is I'm a devout Orthodox Christian and I'm getting spells of uncontrollable sexual impulses which I did not get with so much frequency before and I end up sinning. Before when meds were stopped, I did get extreme sexual highs and heights of uncontrollable sexual euphoria even though I was depressed. But otherwise I do not have such problems like when I used to be on risperidone for years, except I cannot tolerate atypicals. I'm also getting very depressed right now and have feelings of marrying a foreign girl singer I listened to online once singing classical choral music and I have not even met her so it feels a bit deluded to have these feelings. And every time I sin, I get a tendency to hurt myself because I feel I have been infidelous.

I would like to know if another standard AP would be better for 1)the imperfectly controlled rage impulses 2)the sexual impulses and 3)the breakthrough depression. And also I get chronic sleep reversal, I cannot sleep at night but in the day.

pls advise.

i'm not familier with the older traditional antipsychosis medications, but i do know that stelazine is very strong, and they used to use it the 50's for anxiety, and hysetria (old term for over reacting, extremely dramatic, panic attacks)

this is a list of older antipsychosis medications:

they do work very well but they have more side effects, sometimes can block dopamine more potently than new ones today

there's also something called Navane, it's in a class diffrent from others, but you may look into it. Also maybe a benzo like ativan can be good taken with AP medication

Do you take any antidepressants with your currently meds?

Have you discussed the hyper-sexual symptoms with your doctor? Some medications reduce libido, so there may be something to help.

Have you tried counseling? Maybe it could help you with the psychic turmoil and guilt you feel over your symptoms. They're not your fault.

Also, please be aware that Lou Pilder is a notorious troll on this site. You might consider simply not reading his replies.

Navane (thiothixene) is an excellent suggestion. I am wondering if adding Klonopin (clonazepam) would help.

I was going to suggest adding Trileptal (oxcarbazepine), but you mentioned that you found Tegretol (carbamazaemine) to be intolerable. Trileptal can work well to aid in impulse control and prevent rage-attacks.

You describe your being intolerant of a great many drugs. Can you be more specific in your descriptions of what your negative reactions were to each drug?

Have you tried Saphris (asenapine) yet? I have seen a combination of Navane + Saphris work well for someone who has schizoaffective disorder. She is in remission.

Doctor has already added Klonopin 0.5mg to take as required, two days ago. I took two tablets day before to help with sleep. I had suggested Saphris to doc once and she said she feels that it is not a drug to give for me.

I will consider thiothixene. As for your question about my difficulties with other drugs, let me just say that I have gone through a lot, from very painful muscle problems from dystonia to tightening of esophagus and constipation to severe burning sensations in my head and feelings of lava floating in my body (on ris and olanzapine resp.) and disturbed thoughts and existential crises feeling everything was evil (on quetiapine) to severe akathisia (on multiple drugs). I don't have the mind right now to go into specifics. And many which I mentioned like the mood stabilizers imperfectly controlled symptoms in the first place and did not prove useful. And more to the point, the goal should also be to get rid of the neglect inside me that prevents me from doing any sort of useful work which none yet have done so even acceptably.

I've posted here before some time ago and I like your and other's insight. Thank you SLS.
